Background

The 8D elements became increasingly common as quality practices developed over the decades and were compiled in the U.S. Department of Defense’s MIL-STD-1520C. The elements were structured further by Ford Motor Co.’s Global 8D and “Team Oriented Problem Solving (TOPS)” manual. This well-established process is designed to resolve complex problems by focusing on root causes and implementing permanent corrective actions.

Overview

8D consists of eight disciplines, plus an additional pre-step:

  • 0. Plan: Prepare for the 8D process.
  • 1. Establish a team: Assemble a cross-functional team with the necessary skills.
  • 2. Describe the problem: Define the problem clearly, including its impact and scope.
  • 3. Develop interim containment actions: Implement temporary measures to protect the customer.
  • 4. Determine root causes: Use tools such as five whys analysis and a fishbone diagram to identify the root cause.
  • 5. Choose and verify permanent corrective actions: Select and test solutions that address the root cause.
  • 6. Implement permanent corrective actions: Apply the verified solutions.
  • 7. Prevent recurrence: Modify processes to prevent the problem from recurring.
  • 8. Recognize the team: Acknowledge the team’s efforts and contributions.

8D offers numerous benefits, making it a vital option in quality improvement initiatives. It provides a structured approach to problem solving, ensuring all necessary steps are followed methodically.
